Release of Liability and Assumption of Risk Agreement

 

I, the undersigned, hereby acknowledge and agree to the following terms and conditions as a participant in the art class offered during the retreat:

 

1. Voluntary Participation:

I understand that my participation in the art class is entirely voluntary.

 

2. Assumption of Risk:

I recognize that participation in art-related activities may involve certain risks, including but not limited to:

• Use of art supplies and materials (e.g., paint, sharp tools, adhesives)

• Potential for spills, stains, or other minor accidents

• Possible physical strain during prolonged activities

 

I accept full responsibility for any injury, loss, or damage that may occur as a result of my participation, whether caused by my own actions or the actions of others.

 

3. Release of Liability:

I release and hold harmless Reset Holistic Retreats, Amy White and/or Catherine Ford, its employees, instructors, agents, and affiliates from any and all liability, claims, or demands that may arise in connection with my participation in the art class, including any physical injury, property damage, or loss.

 

4. Use of Materials and Supplies:

I understand that I will be using art materials and supplies provided by [Amy White and/or Catherine Ford]. I agree to follow all safety instructions and use materials responsibly.

 

5. Medical Emergencies:

In the event of a medical emergency, I authorize [Amy White and/or Catherine Ford] to seek emergency medical treatment on my behalf. I agree to bear any costs incurred for such treatment.

 

6. Photography and Media Release (Optional):

I, the undersigned, herein authorize Amy White DBA RESET HOLISTIC RETREATS to use, edit, copy, exhibit, distribute and use any video or videos of me for any lawful purpose. The authorization I provide herein extends to all languages, formats, and media that have been discovered now or will later be discovered. The term of this authorization is indefinite and will be active unless I revoke it in writing. Hereby, I also waive my right to approve and/or inspect a product where I appear, or my likeness appears. I also agree and state that I have been compensated for this use of my likeness and/or have agreed to these Video Release conditions without compensation. I hereby waive all and any rights to all royalties or any other compensation in any form when it comes to the use of the video or videos indicated in this form. I consent that the materials will be the sole property of Amy White and shall not be returned to me. I release Amy White from any liability, petitions, and causes of actions caused by me or by my heirs, executives or any other party and will hold him/her harmless. 

 

7. Acknowledgment of Rules:

I agree to follow all rules and instructions provided by the instructors and organizers during the event.
